<건강조언> 젊은 여성이 안드로겐형 탈모가 생기는 이유

최근 대중교통을 이용하면 의외로 젊은 여성들이 정수리 밀도가 떨어져 있는 것을 흔하게 볼 수 있다. 불과 30년전만 하더라도 탈모는 남자들만의 고민이었다. 물론 50대 이후의 여성들이 안드로겐형 탈모 때문에 병원을 방문하는 경우는 많지만 요즘은 젊은 20대 여성들도 안드로겐형 탈모가 증가하는 추세이다.

 

남성탈모의 대부분을 차지하는 안드로겐형 탈모는 DHT호르몬 때문에 발생한다. 남성호르몬인 테스토스테론(testosterone)이 5알파 환원효소에 의해 DHT로 전환되고, DHT가 모유두 안드로겐수용체와 결합하면 모근세포 파괴물질이 분비되어 앞머리와 정수리 모발이 서서히 가늘어지고 짧아진다. 반면 측두부와 후두부의 모발은 영향을 받지 않는다. 

 

하지만 이러한 진행 양상은 남성들에게 해당되며 여성 안드로겐형 탈모는 헤어라인은 비교적 잘 유지가 되고 정수리 가르마를 중심으로 크리스마스 트리처럼 점점 가늘어지는 특징을 보인다. 그 이유는 여성의 경우 아로마타제(aromatase)라는 효소가 앞머리 헤어라인 쪽에 많이 분포되어 있어 테스토스테론을 DHT로 만들지 않고 에스트라디올(estradiol)이라는 여성호르몬으로 전환시켜 주기 때문이다. 똑같은 안드로겐형 탈모라도 남성과 여성의 탈모형태가 달라지는 이유이다.

 

젊은 여성에게는 안드로겐형 탈모가 잘 발생하지 않는다. 그 이유는 여성호르몬 때문이다. 여성호르몬은 모발생성을 촉진하는 호르몬으로 여성호르몬이 왕성하게 분비되는 젊은 여성들에게는 증상이 잘 나타나지 않고 여성호르몬이 현격히 줄어드는 시점인 40대 중후반에서 많이 발생한다.

 

하지만 최근 젊은 20대 여성들에게 안드로겐형 탈모가 증가하는 대표적인 원인으로 다낭성 난소 증후군이 있다.

 

다낭성 난소 증후군(polycystic ovary syndrome)이란 무배란성 월경 이상과 난소에 여러 개의 물혹이 생기는 증상 또는 다모증을 동반하는 질환을 말한다. 다낭성 난소 증후군은 남성호르몬인 테스토스테론 생성이 증가하고 이로 인해 DHT로 전환되는 양도 증가하게 되어 안드로겐형 탈모가 발생한다. 다모증이 동반되는 이유는 DHT가 눈썹 아래 신체의 체모는 오히려 성장시키는 특징이 있기 때문이다. 따라서 젊은 여성이 안드로겐형 탈모가 진행되었다면 산부인과를 방문하여 다낭성 난소 증후군 검사를 받는 것이 필요하다. 또한 비만인 경우 고안드로겐혈증의 임상 증상이 더 악화될 수 있으므로 적정체중을 유지하는 것이 무엇보다 중요하다.

 

그 외 여성의 사회적 활동이 늘어나면서 증가하는 스트레스가 탈모유전자를 발현시켜 안드로겐형 탈모를 유발할 수 있다. 스트레스는 5알파 환원효소와 안드로겐수용체를 활성화시켜 여성일지라도 안드로겐형 탈모를 조기에 발생시키기도 한다.

 

하지만 탈모가 있다고 낙담하기 보다는 정확한 원인진단을 통해 다낭성 난소 증후군이라면 산부인과 치료를 병행하고 스트레스로 인한 것이라면 최대한 긍정적인 마음가짐과 함께 탈모치료를 하면 매우 효과적으로 탈모를 극복할 수 있다.
