【한국건강관리협회 】오진규 가천대학교 길병원 비뇨의학과 교수=백세 시대를 바라보는 21세기 대한민국에서 ‘전립선’이라는 용어가 더 이상 생소하지만은 않다. 남성들에게만 있는 장기인 ‘전립선(prostate)’은 ‘앞’을 의미하는 ‘pro’와 ‘서다’는 의미의 ‘state’가 결합하여 이루어진 단어로, 그 뒤에 ‘샘’을 의미하는 ‘gland’가 붙어서 방광의 아래에 밀착된 부생식선을 일컫는다.
전립선암, 남성 암 중 절대 비중
인간의 모든 정상 세포들은 각자의 수명을 다하면 사멸하지만, 일부 세포들은 죽지 않고 증식을 거듭하면서 종괴를 형성하기도 한다. 그중 세포 증식이 매우 빠르고 다른 장기로 퍼져 나가며 생명을 위협하는 종양을 악성 종양 혹은 암이라고 부른다. 전립선 역시 이러한 암 세포의 발생으로부터 자유로운 장기는 아니며, 전립선 조직에 생기는 암을 ‘전립선암’이라고 한다. 남아프리카 공화국 최초의 흑인 대통령이자 세계인권운동의 상징인 넬슨 만델라, 중국의 국가 주석 덩샤오핑, 프랑스의 전 대통령 미테랑, 일본의 천황 아키히토까지 수많은 각국의 유명 인사들이 전립선암으로 투병했을 만큼, 전립선암은 남자라면 누구에게나 찾아올 수 있는 어두운 그림자라 할 수 있다. 우리나라 역시 예외가 아니어서 2019년 우리나라의 국가 암 통계에 따르면 전립선암은 전체 암 중 7번째로 많이 발생하고 있으며, 남성 암 중에서는 무려 4위를 차지할 만큼 절대적으로 많은 비중을 차지하고 있다.
암세포를 완전히 제거할 수 있을까?
안타깝게도 초기의 전립선암은 별다른 증상이 없는 것으로 알려진다. 환자 대부분이 중년 남성에게서 흔히 나타나는 다양한 배뇨 증상, 예를 들어 소변이 자주 마렵다든가 소변 줄기가 최근 가늘어진 것 같다든가 아니면 소변을 본 말미가 개운하지 않다는 등 전립선암과는 동떨어진 불편감을 호소하며 진료실 문을 두드린다. 전립선 증상이 확인되면 40세 이후의 남성들에게는 초기 스크리닝 검사로 전립선 특이항원 검사, 직장 수지 검사 및 전립선 초음파 검사 등을 시행하게 된다. 이러한 검사 소견을 종합하여 전립선암이 의심되는 경우에는 가는 침을 이용하여 전립선내 여러 군데의 조직을 채취하고 이를 병리학적으로 분석하여 나쁜 암세포가 발견되는지 찾는다. 물론 아무 문제가 없으면 가장 좋겠지만 만일 조직검사에서 전립선암 세포가 확인된다면 어떻게 될까? 암세포가 몸 안에 있다고 해서 그것이 곧 내 수명시계의 끝을 의미하는 것은 결코 아니다. 암세포가 생명에 지장을 일으키는 장기로 퍼지지만 않았다면, 충분히 완전하게 제거할 수 있기 때문이다.
기술의 발전이 이룩한 고도의 수술법
전립선암이 진단된 경우 그다음으로 해야 하는 과정은 암세포가 다른 장기로 퍼졌는지 여부를 파악하는 일이다. 전이가 확인되어 전이성 전립선암으로 진단된 경우라면 호르몬 치료 등 약물치료를 우선적으로 시행한다. 그러나 대개 암세포가 전립선 내에 국한된 국소성 전립선암으로, 과거에는 전립선의 위치가 우리 몸속 중 깊은 곳에 있고 출혈이 심하여 완전한 제거가 어려웠다. 또한 수술 이후 합병증인 발기부전이나 요실금 증상이 극심하여 90년대까지는 방사선 치료를 선호했다. 이후 90년대 중반부터 전립선 주변의 신경다발을 보존하는 수술식이 세계적으로 활발하게 보급되면서 개복으로 전립선을 근치적으로 제거하는 수술법이 표준 치료법으로 각광 받기 시작했다. 첨단 과학기술의 발달과 의료 술기의 고도화에 힘입어 이제는 사람의 손보다 더 작고 정밀한 로봇을 이용하여 출혈을 최소화하면서 전립선 주변 신경다발의 손상을 최소화할 수 있는 로봇수술이 보편화되고 있다. 아직까지 로봇수술 도구가 우리가 SF 영화에서 보듯 로봇이 다 알아서 수술을 한다거나 자동차의 자율주행기능처럼 의사가 손 놓고 편안하게 지켜보는 수준은 아니다. 그러나 로봇을 이용한 전립선 절제술의 발달로 기존 수술법에 비해 수술 중 출혈이 현저히 적고, 좁은 공간임에도 작은 기구들을 불편함 없이 조작할 수 있어 보다 안전한 수술이 가능하다. 또, 수술 이후 발기부전이나 요실금 등과 관련된 신경다발을 최대한 보존할 수 있게 되었다.
전립선암을 슬기롭게 예방하는 방법
다행히도 금연, 금주를 실천하고, 적절한 운동과 동물성 지방 섭취를 줄이고, 고섬유질 식이를 하면서 토마토나 카레 등을 즐겨 먹는다면 이러한 전립선암의 발생을 억제시킬 수 있다고 알려진다. 아울러 가족력이 있다면 40세 이후에 정기적인 비뇨의학과 검진을 통해 조기에 전립선암을 찾아낼 수 있고, 설령 느닷없이 암을 진단받더라도 100세 장수에는 전혀 문제가 없는 상태로 치료가 가능하니 전혀 불안해할 필요가 없을 것이다. 모든 암이 그렇듯 전립선암 역시 조기에만 발견된다면 두려울 것이 없기 때문이다.
